Study Specific Report (formerly the Study Specific Financial Disclosure form)
A Study Specific Report must be submitted with each new grant proposal and or research protocol (both human and animal subjects) application. The purpose of the Study Specific report is to determine - for each study - whether there is a financial interest that needs to be managed.
A new form must be completed at the time of Continuing Review and/or Renewal and at any time thereafter if there is a change in circumstances once the project has begun.
Remember, these disclosures are personal and confidential; therefore each person on the project must complete and submit his/her own report. As of April 1, 2010 all WCMC employees will be required to complete the 2011-2012 Annual Survey before being permitted to complete a Study Specific Report. All Study Specific Reports will be generated directly from the new system. Please use the help guide to walk through the process of printing the report to PDF in order to forward to the appropriate office.
Completing a Study Specific Report
To complete a Study Specific Report visit the new Conflicts Management System and sign in with your WCMC user name and password. If you have not yet created an Annual Survey you must do so first by following the on-screen prompts. Once your Annual Survey has been completed click on the button labeled "Create Study Specific Report."
Paper copies of the Study Specific Report will only be given to investigators who are not WCMC employees. If an investigator is not a WCMC employee, he/she will have to request a paper copy by contacting Amenophis Faussett at (646) 962-8199 or conflicts@med.cornell.edu
