Speakers must submit 50 glass slides as well as a single paper copy and an electronic copy (sent on CD with the slides or via email) of the case submission form (PDF) with your registration form by August 22, 2008.

For all PDF documents: 
Prior to the meeting, all presenters will receive a slide set consisting of slides for all cases with case histories. Additional slide sets are available for purchase at a cost of $40 each contingent on availability.
