| All users will log on to the survey the same way they would log onto their Eudora accounts. |
| What does the survey consist of? | |
| The survey is in 2 parts. Part I consists of entering your personal contact information (questions #1-8) and three Sub Parts (A, B, & C). Then there are 10 Yes/No questions. If you answer "No" to all of the questions, then the survey is complete and you are done with this requirement for the current Fiscal Year. Those employees with an NYPH relationship who answer "Yes" to question #18 will have to continue and answer questions #19-24 and supply details where indicated. Within minutes of completing the Part I survey you will receive a confirmation email. If you answered "Yes" to questions #9-17 or #19-24 you will be sent the link that takes you to Part II where you will be asked to enter the names (entities) and details of your external financial relationships. |
| What is a "Financial Entity"? | |
| The term "Financial Entity" refers to any entity, financial or not, including, but not limited to, any for-profit business, not-for-profit organization, or governmental agency. A relationship with any entity that might present a conflict of interest or commitment should be disclosed. |
| What if I am a member of the faculty involved in giving lectures at multiple institutions for less than $2,000 honoraria per lecture? | |
| Instead of having to create an entry for each of the institutions for which you lecture, you may reply by using the term "Various Institutions" in the "Entity Name" field on Step 1, the "Basic Disclosure". Then, in Step 2, edit the entity called "Various Institutions" and in the first "Explanations (optional)" text box, in the "Entity Details" tab, enter the list of institutions and the amount per institution. If more space is needed, the list can continue in the second "Explanations (optional)" text box slightly further down on the same page. Please note that under the "Person's Relationship to this Entity" section, the check box is defaulted to "Consultant": this answer is correct for this situation. Be sure to answer the questions listed on the page for all of the institutions as a group, not individually. |
| When trying to log on to the survey, my browser displays a series of Security pop up windows. What should I do? | |
| Security pop ups are displayed according to the security settings each individual has chosen for his or her particular browser. To access the survey, simply click "OK" on each of the security pop-ups and your submission will reach the Conflicts Management Office correctly and on time. |
