“Work” Rounds. . The first team activity of each day is usually work rounds, consisting of the PGY2 resident, two PGY1 residents (interns), and the student(s) assigned to them. You will see and examine each of the patients being cared for by the team. Whenever the team sees patients, you should take the initiative with your patients. In the hall (usually), you will highlight the active clinical issues in a sentence or two. Then, you should be the first to enter the patient's room, greet the patient, ask a few questions, examine the patient, and review a plan for the day. You should plan to return to the patient after rounds to go into more detail.
The Intensive Care Units use a highly organized method of presenting
cases (http://med.cornell.edu/education/curriculum/third/pdf/
care_manual.pdf). I recommend reviewing this format before
starting the Cardiology-MICU rotation.
Attending Rounds. The entire team rounds with the attending physician, discussing 1-2 patients. The emphasis is on the educational and scientific aspects of the cases.
You should present all cases that you admit. The admission oral
presentation should take ≤ 5 min. To achieve this goal,
you should practice the presentation beforehand. You should not
read the presentation; a few notes are acceptable. The review
of systems should be restricted to pertinent positive or negative
findings.
While it is important to present the complete history and physical,
the core of the presentation is the summary,
assessment, and plan.
Chart Rounds. In the afternoon,
the PGY2, PGY1 and student meet to review each patient’s
progress, and to make management plans
Sign-out Rounds. You are responsible
for providing the on-call team with a written synopsis of each
of your patients. You should review the sign-out orally with the
on-call team.
Physical Diagnosis Rounds. Thursday
3 PM for students in the General Medicine block. The group will
meet with Dr Yaghoobzadeh on Greenberg 5 at the nursing station.
You will see patients as a group and demonstrate physical examination
findings.
Radiology Rounds. Students in
the Cardiology/MICU block will also participate in weekly seminars
devoted to clinical radiology. These occur every Thursday at 3
PM and take place in the Radiology Conference Room on Starr 8.
