Medicine Electronic Case Log
We ask you to keep a log of the core cases that you encounter. The purpose of the case log is to document your encounters with the key clinical cases and problems in the clerkship. This can be helpful both to you, the student, for assessing your experiences in clinical medicine, and to the medical college, for planning the clinical curriculum.
The case log uses a web format. Whenever you work with a patient with one of the core problems or diagnoses listed below, please log on and record the key clinical information. The central encounters are:
- Abdominal pain
- Anemia
- Asthma/COPD
- Chest pain
- Common cancers
- Congestive heart failure
- Dyspnea
- Fever
- Fluid, electrolyte, acid-base disorders
- Gastrointestinal bleeding
- Liver disease
- Pneumonia
- Renal insufficiency
- Respiratory failure
- Shock/sepsis
In addition, please record the key procedures that you perform. These are:
- Arterial blood gas
- Intravenous line placement
- Lumbar puncture
- Nasogastric lavage
- Paracentesis
- Phlebotomy
- Thoracentesis
- Urinary catheter insertion
- Wound suturing
Each entry should take less than 5 minutes to record. You can record a given type of encounter as many times as it occurs. Thus, if you see five patients with chest pain, you can enter each one. You can also go back and edit your entries, and you can keep your own record of your experiences.
